DVLA medical addresses: where to send each letter
Short answerIt depends on the letter. Further medical evidence goes to DM Further Evidence, DMI Support, D5, DVLA, SA99 1DL. Postal reapplications go to Drivers Medical Group, DVLA, Swansea, SA99 1TU, and a notice of appeal to Drivers Medical Group, Swansea, SA99 1DF. Subject access requests and complaints have their own addresses, below.

DVLA’s medical letters don’t all go to one place. GOV.UK gives a different address for further evidence, a reapplication, a notice that you’re appealing, a subject access request and a complaint. This page lists each one as GOV.UK published it in October 2026, with the page it comes from, so you can check it on the day you send.
We’re not DVLA. DL Guide is an independent publisher. We can’t pass anything on to DVLA, look up a case or tell you how an application is progressing: only DVLA can.

A complaint about how your case is being handled

Forms that carry their own address
- Telling DVLA about a condition. GOV.UK says car and motorcycle drivers can report their condition online when they check it. Bus, coach and lorry drivers send a form or questionnaire to DVLA, and the address you need is on the form. Our guide to DVLA medical forms by condition lists them.
- Surrendering your licence for a medical reason. GOV.UK says to send the declaration of voluntary surrender (car or motorcycle) or form VOC99/CERT (bus, coach or lorry) with your licence, and that the address is on the form. Email, phone and webchat
The only DVLA email address the kit gives is the subject access address above, and GOV.UK asks people not to use it for general enquiries. For a question about a medical case, GOV.UK’s Contact DVLA service finds the right phone number, email or postal address, including for health conditions and driving. DVLA’s complaints procedure says contacting the team dealing with your case is often the quickest way to resolve a query, so start there before you complain.
Before you post anything
- Check the address on the GOV.UK page on the day you send: DVLA addresses change.
- Put your reference number, driver number and date of birth at the top of every letter.
- Send copies, never originals. Use a tracked, signed-for service, keep the receipt and note the date.
- If you’re helping someone, the driver signs, or you enclose their signed authority for you to act for them.
Northern Ireland
Licences in Northern Ireland are handled by the Driver and Vehicle Agency (DVA), not DVLA, so none of these addresses applies there.
